    Are you looking to apply your strong clinical skills on a more personal level? Do you enjoy working independently, as well as having team support one call away?  The Triage RN position is ideal for the clinically strong nurse who enjoys coordinating direct and indirect patient care by assessing each call and determining if a visit is needed or if assistance can be handled over the phone. There is a high degree of collaboration with patients, family caregivers, physicians, facility partners and the Agrace care team.



    This position plays a key role in enhancing quality of life for patients and families facing serious illness. This position focuses on patient and family centered care consistent with the philosophy, goals and standards, of Agrace, as well as support with immediate pain and symptom management, education, teaching, nursing assessments, interventions, evaluations, documentation and coordination of care.  Excellent oral and written communication skills are necessary to be successful in this position.


    Position Details:

    • .7 FTE (28 hours/week) + opportunity to pick up additional shifts if wanted
    • Hours: 4pm - 12:30am and 3pm - 11:30pm (start times vary throughout the week)
    • Works an every other weekend rotation
    • Benefits eligible with great paid time off plan














    • Associate degree of Nursing (Bachelor’s degree preferred)
    • Current, unrestricted Wisconsin RN license
    • One year direct patient-care nursing experience, preferably in ICU, ER, Medical-Surgery, Oncology or Palliative Care or two years relevant health care experience.
    • Valid driver license and car insurance and reliable access to an automobile in good operating condition.
