    Are you looking to apply your strong clinical skills on a more personal level? Would you like to get away from an every-other weekend rotation? Do you enjoy being out in the community and starting and ending your day at home?

     

    The Admissions Nurse plays a key role in enhancing quality of life for patients and families facing serious illness. This person meets the incoming hospice patient and family at a very important time, providing them much needed reassurance and education, as well as support with immediate pain and symptom management, and then coordinates a smooth transition of services to the care management team.

    Responsibilities

    This role is ideal for the experienced nurse who enjoys working independently with a variety of patients and diagnoses, travelling locally to a variety of settings, such as patient homes or health care facilities. There is a high degree of collaboration with patients, family caregivers, physicians, facility partners and the Agrace care team.

     

    The regular schedule can be either two 8-hour (10A-7P) and two 12-hour (9A-10P) shifts per week OR five 8-hour (10A- 7P) shifts per week.  Either schedule requires every 4th weekend, 1-2 holidays per year and no on-call responsibilities. 

    Qualifications

    • Associate degree of Nursing (Bachelor's degree preferred)
    • Current, unrestricted Wisconsin RN license
    • One year direct patient-care nursing experience, preferably in ICU, ER, Medical-Surgery, Oncology or Palliative Care or two years relevant health care experience, preferred
    • Valid Wisconsin driver license and car insurance and reliable access to an automobile in good operating condition.
