    For more than 35 years, Agrace has been a nonprofit, community based health care agency dedicated to providing expert physical, emotional and spiritual support to patients and families through the stages of serious illness. Agrace HospiceCare is nationally recognized for the comprehensive hospice services we provide in patients' homes, the Agrace HospiceCare inpatient unit and skilled nursing and assisted living facilities throughout south central Wisconsin and northern Illinois.

    Responsibilities

    The Enrollment Representative meets with prospective patients and families in their place of residence with the goal of enrolling patients and educating them about Agrace values, services, benefits and philosophies. The Enrollment Representative answers questions regarding enrollment criteria, hospice services, and referral requests. They also help the patient and family complete enrollment documents. This position will be cross trained to support the Enrollment Coordinator team, an office-based position which coordinates referrals over the phone and electronically. 

     

    This a full time, benefit eligible position. Regular working hours are 10:00 AM to 7:00 PM with every 4th weekend rotation required.

    Qualifications

    • High School diploma or equivalent, required
    • 1 - 3 years’ customer service experience in healthcare setting, required
    • Demonstrated experience handling difficult conversations with patients and families, required
    • Valid Wisconsin driver license and car insurance and reliable access to an automobile in good operating condition, required
