    Responsibilities

    The RN Hospital Liaison serves as the primary liaison between Agrace and the designated hospital(s). This person assists the referring physicians, discharge planners and other healthcare professionals in facilitating the transition of patients and families to the appropriate post-hospital setting. This position will be responsible for assessing and evaluating patients for hospice and/or palliative care eligibility and educating patients and families on Agrace philosophies, benefits and services.  

     

    Specifics of position:

    • Full-time position 1.0 FTE
    • Monday - Friday 9am - 6pm
    • Works every 4th weekend

    Qualifications

     
    • Bachelor's degree in Nursing
    • Certification/Registration/Licensure: Current, unrestricted Wisconsin RN license
    • One year direct patient-care nursing experience preferably in ICU, ER, Med-Surg, Oncology or Palliative Care or two years relevant health care experience
    • Computer experience preferred (required knowledge and skills within 90 days of taking position)
    • Skill and comfort level in developing relationships, with sales/marketing experience a plus
    • Valid Wisconsin driver license and car insurance. Must have access to an automobile in good operating condition.
