If you are looking to provide one-on-one, patient-focused care with a dedicated and collaborative team, the RN Support at Agrace is a great fit for you! The RN Support covers for any nursing needs throughout a designated service area as directed by the Clinical Team Manager. This includes, but is not limited to coverage for the RN Case Management and RN direct care positions.
The RN Support coordinates direct and indirect patient care that is based on nursing process using an interdisciplinary team approach. The RN Support maintains accountability for nursing practice and for all aspects of nursing care delegated to other personnel within their scope of practice. The RN Support is responsible for nursing assessments, interventions, evaluations, documentation and coordination of care. This position focuses on patient and family centered care consistent with the philosophy, goals, standards, regulations and policies of the program and Agrace in order to meet our mission of enhancing quality of life for patients and families facing serious illness.
