The Admissions Nurse plays a key role in enhancing quality of life for patients and families facing serious illness. This person meets the incoming hospice patient and family at a very important time, providing them much needed reassurance and education, as well as support with immediate pain and symptom management, and then coordinates a smooth transition of services to the care management team.
This role is ideal for the experienced nurse who enjoys working independently with a variety of patients and diagnoses, travelling locally to a variety of settings, such as patient homes or health care facilities. There is a high degree of collaboration with patients, family caregivers, physicians, facility partners and the Agrace care team.
The regular schedule can be either two 8-hour (9A-6P) and two 12-hour (9A-10P) shifts per week OR five 8-hour (10A- 7P) shifts per week. Either schedule requires every 4th weekend, 1-2 holidays per year and no on-call responsibilities.
