    Well known in the community for  providing excellent end of life services, Agrace has an exciting leadership opportunity to join a team of dedicated health care professionals that pride themselves in providing quality resident care.  

    Responsibilities

    The Clinical Team Manager (CTM) for the Janesville Inpatient Unit, under the supervision of the Director of Clinical Services is based in Janesville and provides leadership, guidance and direct supervision of the inpatient team on a day-to-day basis. This position ensures hospice services are patient and family centered and consistent with the philosophy, goals, standards, regulations and policies of the program and Agrace.  Responsibilities include coordinating direct and indirect patient care using an interdisciplinary team (IDT) approach within a complex and changing environment.  The CTM maintains accountability for quality outcomes, financial performance, and the overall growth and development of the team. The Clinical Team Manager promotes a positive and innovative culture through coaching, facilitation, decision-making, problem-solving, census building, and regulatory and clinical expertise. 

    This could be the ideal role for an experienced and decisive RN leader, well versed in health care delivery systems, who takes pride in mentoring and engaging their teams through open sharing of information.

    Qualifications

    Minimum Qualifications include: 
     

    1. Associate’s Degree in Nursing (Bachelor's Degree preferred) or Masters in Social Work (MSW)
    2. Certification/Registration/Licensure: Current, unrestricted Wisconsin RN license; CPR Certification preferred (required within 90 days of hire) or current WI SW license, if applicable.
    3. Five to seven years relevant registered nursing, preferably in complex healthcare or hospice environment.
    4. Five years progressive supervisory and/or preceptor experience
