Clinical Team Manager (RN) - Homecare

Well known in the community for  providing excellent end of life services, Agrace has an exciting leadership opportunity to join a team of dedicated health care professionals that pride themselves in providing quality resident care.  

The Agrace Clinical Team Manager (CTM) position provides you an opportunity to utilize your great clinical management experience leading others in a positive, innovative culture. This position helps to meet our mission of enhancing quality of life for those facing serious illness by supervising a a full interdisciplinary team.  In addition, this role is accountable for quality outcomes, financial performance and overall growth and development of the team.

The ideal candidate will be collaborative, have strong relationship building skills and have excellent clinical expertise. Hours for this position are generally 8am-5pm Monday- Friday, but can vary based on after-hours management needs.  

To qualify, you must have:
 

• Associate’s Degree in Nursing required with proven clinical management experience. Bachelor’s Degree in Nursing strongly preferred.
• Certification/Registration/Licensure: Current, unrestricted Wisconsin Registered Nurse license, CPR Certification preferred (required within 90 days of hire) or current WI SW license, if applicable
• Five to seven years relevant RN or medical SW experience, preferably in complex healthcare or hospice environment.
• Five years progressive supervisory and/or preceptor experience
• Valid Wisconsin driver license and car insurance and reliable access to an automobile in good operating condition.
