The APNP is a member of the medical services team and the palliative care consultative team. This position participates in the assessment, planning, implementation and evaluation of clinic and community-based palliative care interventions, as well as some coverage at our Inpatient Unit (approx. one weekend rotation every other month). This position is responsible for interdisciplinary collaborative practice, ongoing quality assessment and performance improvement, performance management, financial management, and ensuring exceptional care delivery consistent with hospice/palliative care philosophy and best practice. 



  1. Current WI RN license, Advanced Practice Nurse Prescriber licensure, and NP certification.
  2. Master’s Degree in Nursing
  3. Hospice or palliative care experience and/or certification preferred
  4. Experience with and knowledge of health care policies and coding/reimbursement practices, state and federal health care regulations, standards and procedures and home health and hospice certification and accreditation preferred
  5. CPR certification preferred (required within 90 days of hire)
  6. Valid driver's license and access to an automobile in reliable operating condition
