Overview

Do you want to spend quality one-on-one time with your patients? Our Continuous Care might be the place for you! Agrace’s Licensed Practical Nurses (LPN) role travels locally and works in both patient homes and health care facilities. The LPN - Continuous Care will provide patient care as identified in the Plan of Care, delegated by the RN for Continuous Care patients or other Agrace patients (Home Care or IPU), as needed.

Responsibilities

This position focuses on patient and family centered care consistent with the philosophy, goals, standards, regulations and policies of the continuous Care program and Agrace.  Continuous Care is short-term care for an individual and their family/caregivers in a home or facility where intensive skilled nursing care is needed to control patient symptoms, such as severe pain, breathing difficulties or other urgent or complex health problems. Continuous Care patients have a detailed care plan that defines the interventions needed during acute care.

The ideal candidate will be flexible, independent and patient and family focused. Our Continuous Care LPNs work 12 hour shifts from 7pm - 7am. This .9 FTE position (32 hours/week) will work three 12-hour shifts/week and this will include an every other weekend rotation. 

Qualifications

To qualify, you must have:

  • Graduation from an accredited program for Licensed Practical Nurses
  • Certification/Registration/Licensure: Current, unrestricted Wisconsin LPN license
  • CPR Certification preferred (required within 90 days of hire)
  • 1-2 years relevant experience preferably in hospice, oncology or ICU preferred
  • Ability to communicate effectively verbally and in writing.
  • Ability to be flexible and effectively manage priorities, multiple tasks and meet deadlines.
  • Ability to maintain patient confidentiality.
  • Valid Wisconsin driver’s license and car insurance.  Must have access to an automobile in good operating condition.
